A 5 kVA transformer is nearly always single phase, used for control power or a small dedicated circuit. 480 to 120 volts is the most common configuration, feeding contactor coils, instrumentation and a service receptacle inside a panel.
At 480 to 120 volts this delivers about 42 amps on the secondary. Size it on inrush rather than running load, because contactor coils draw several times their holding current when they pick up.
